Website Form With Advanced Features

I need a custom electronic form built for my website. The form must support file uploads, use conditional logic to show or hide fields based on a user’s previous answers, and send real-time email notifications to both the user and me when it is submitted.

Please code the form so it can be embedded easily on an existing HTML/PHP page and styled with my site’s CSS. A simple back-end script (PHP, Node, or similar) to process the submission and handle the email routing is acceptable as long as it remains lightweight and secure.
