Qualified specialist experienced in UK clinical negligence cases.
I need a medical report for you to provide an independent medical report review concerning my potential personal injury claim against Bluecrest Health Screening.
Before we proceed, I would like to clarify the scope of your proposed work and your relevant clinical and medico-legal experience.
My case concerns a health-screening assessment on 15 January 2025, at which my recorded glucose result was 7.5 mmol/L, followed by hospital admission on 22 January 2025 with severe hyperglycaemia, elevated ketones, a markedly raised HbA1c and diagnoses including type 2 diabetes with ketosis and acute kidney injury.
The principal questions are whether the screening provider should have taken further action in relation to the result and, if so, whether appropriate action would probably have led to earlier treatment and prevented or reduced the severity of my subsequent illness or complications.
Could you please confirm:
1. Your professional qualifications, relevant clinical experience and current role.
2. Your specific experience in diabetes, endocrinology, acute medicine or clinical negligence.
3. Whether you can independently assess breach of duty and medical causation, or whether your service is limited to medical research, literature review and manuscript preparation.
4. Whether you have experience preparing medico-legal expert reports for civil proceedings in England and Wales.
5. Your proposed deliverable, fee and timescale, including whether you recommend an initial review before a full report.
I need to ensure that the person providing the opinion has the appropriate clinical expertise for the issues in dispute.
