PDF Registration Forms to Google Docs

I have a small batch of registration forms—no more than five in total—that were originally supplied as static PDFs. I need each one re-created in Google Docs so the text, fields, and basic layout match the originals as closely as possible while remaining fully editable.

Accuracy is essential: every input field, checkbox, and line of instructional text on the PDFs should appear in the new documents and align cleanly on the page. Consistent font use, proper spacing, and a tidy page break where necessary will make it easy for my team to maintain the forms later.

Deliverables
• Up to five individual Google Docs, each mirroring its corresponding PDF registration form
• Shared links with full editing rights
