Fix Snow Quote Calculator

The quote calculator on my website stopped working. Customers pick their driveway size and plan, press Get My Quote, and nothing happens. It's supposed to show the price underneath.
It worked fine all summer. Snow removal bookings are starting now so I need it fixed quick.
It's a plain website, not WordPress. I can send you the files, but I'm not giving out my hosting login.
Looking for someone to:
find out why it's broken and fix it
make sure the prices come out right
tell me how to put the fixed file back up
