I have a backlog of clinical notes that need to be transformed into clean, well-formatted text so they can be uploaded to our electronic health record. The source material is audio dictated by physicians; the total volume sits somewhere between 100 and 500 pages once transcribed.

Because accuracy and confidentiality are critical, I’m looking for someone who is already comfortable with medical terminology and common charting conventions. You’ll be receiving encrypted audio files in batches, then returning the finished notes in Word or RTF, formatted with the standard headings we already use (CC, HPI, ROS, PE, Assessment, Plan). Turn-around on each batch will be two business days or less so the clinic can keep pace with patient follow-up.

Deliverables
• Fully transcribed, proof-read clinical notes (Word/RTF)
• Consistent formatting that matches our template
• HIPAA-compliant handling of all audio and text files

I will start by sending a small test batch to ensure we’re on the same page with style and accuracy. Once approved, the remaining files will follow on a rolling basis until the project is complete.
