I already have four hosted demos that combine a simple webpage, a Google Form, and its linked Google Sheet. The “Search” flow behaves exactly as intended: once the user submits, an Apps Script builds a nicely formatted email and sends it to the right people.

The two sister flows—“Machine Failure” and “Hiring”—share the same code base but their emails misbehave. They either fail to send or drop key merge fields. I need those two forms to mirror the “Search” behaviour and- while you are in there ‑ update a few phrases inside every template (all three forms use very similar wording, it is literally a couple of line edits).

Next, I built a fresh “Doctor” scenario. The front-end page, Form and Sheet are ready; the only difference is that the user can pick among three options instead of the usual two. I want the same logic you polish on the first set ported here so the email routing, subject lines and template variables adapt correctly to A, B or C.

Everything lives in the same Google Workspace, so you will work inside Apps Script attached to each Sheet. When done I expect:
• “Machine Failure” and “Hiring” forms sending clean, accurate emails identical in style to “Search”.
• Text tweaks applied across all three existing templates.
• “Doctor” form fully wired with the three-option logic and verified by test submissions.
