Retype PDF Tables to Excel

I need several PDF files that consist solely of tables and embedded form sections converted into a fully editable Excel workbook. Every element in the original—text, numbers, column headings, checkbox positions, signature lines, and general formatting—has to be reproduced with pixel-level accuracy.

Please mirror the table layout exactly so I can drop the new sheets straight into our existing reporting process without additional cleanup. Where the PDFs show checkboxes or signature areas, recreate them using Excel form controls or another clear, interactive method that keeps the sheet functional.

Deliverables
• One Excel file (or one per source PDF, if simpler) whose sheets match each original page.
• No data loss or substitution; spelling, figures, and formatting must be identical.
• Printable view that lines up with the source when printed at 100 %.
