QR-Enabled Emergency Safety Card

I need help turning a simple idea into a clear, printable Safety Card that can live both in my car and in my wallet. The card must carry:
• a scannable QR code that holds my chosen emergency contact details
• a short line or two of instructions so anyone who finds the card knows to scan first and call second
Crucially, the family member’s phone number should not be visible on the card itself—only revealed once someone scans the code. A URL, vCard, or other secure method is fine as long as the helper sees the number on their phone screen, not on the physical card.
What I am looking for from you:
• Graphic layout sized for standard wallet credit-card dimensions plus a slightly larger dashboard/visor version for the vehicle.
• Generation of the QR code (or clear guidance on how I can generate it myself in case the data ever changes).
• Print-ready PDF or high-resolution PNG files.
• Quick note on recommended paper stock or lamination so the card survives heat inside a car.
You are free to use Canva, Illustrator, Inkscape, or any other design tool—just provide the final assets and, if possible, the editable source file.
Once I approve the final design, I’ll handle the printing locally, so color specs (CMYK or RGB) and bleed marks should be included.
If you have a clever way to password-protect or otherwise obscure the raw contact data inside the QR, let me know; simple solutions are welcome as long as they’re reliable in an emergency.
Looking forward to seeing a clean, minimal design that can genuinely help when seconds count.
