PowerApps IT Request Form Setup

I need a Power Apps form that lets staff submit and track IT support or access requests, with all data saved neatly in a connected SharePoint list. The form should be clean, responsive, and easy for non-technical users to navigate, capturing essentials such as requester details, issue or access needed, priority, attachments, and current status.

I already have a SharePoint site available; you will create the list (or adapt an existing one) and wire the form to it so new requests appear instantly and can be filtered or edited by authorized personnel. Basic validation (required fields, attachment size, sensible date ranges) is important, as is a straightforward way for the service desk to update status inside the same app.
