PDF to Word Conversion with Table of Contents

I need a 42-page scanned PDF document converted into an editable Word document. The scanned document is somewhat blurry.

Requirements:
- Images and graphics may be cleaned up or replaced with more clear images
- Correct any typos during conversion
- Add a table of contents
It will need information from the PowerPoint added to it.
I need the PDF to match the PowerPoint that I have attached.
What is marked in Green is simply important information.
What is marked in red is new information that needs to be added to the document.
I have also attached other material that you could possibly use for images or wording usage.

Additional Note:
While there is no specific formatting required, freelancers are encouraged to make the document look more professional. However, it is critical to maintain the same order of information as students will be taking exams after each section.
