Patient Data Entry in Excel

I need patient information from my medical billing records transcribed into an Excel workbook. I’ll supply the spreadsheet template—complete with column headers and validation rules—so your job is to copy the details exactly as they appear on the source documents and keep the structure intact.

What you’ll work with
• Scanned or PDF billing files containing each patient’s full name, date of birth, address, phone, account number, and insurance ID.
• My locked-down Excel template for fast, consistent entry.

What I expect
• Absolute accuracy: names, numbers, and dates must match the originals character for character.
• No blank required fields and no stray formatting.
• Respect for confidentiality and HIPAA-level data handling; files must stay within our shared drive.

Deliverable
A completed Excel workbook populated with all patient records, ready for my billing run. I’ll review using the built-in validation, and once everything checks out the task is done.
