About the Project

Expedient Medicolegal Services is seeking an experienced Jotform implementation and QA specialist to configure seven comprehensive applicant questionnaires used for workers’ compensation medical evaluations.

The questionnaires cover Mental Health, Chiropractic, Orthopedics, Internal Medicine, Dentistry, Ophthalmology, and General Specialty.

This is primarily an implementation project—not a questionnaire-writing or software-architecture project.

Our medical content is already written, and our internal team will provide detailed build specifications covering field structure, question wording, English/Spanish text, conditional logic, field identifiers, validation requirements, PDF mapping, and JSON output.

Your role is to implement those specifications accurately inside Jotform, identify Jotform-specific limitations, troubleshoot problems, and thoroughly test the completed forms.

What You Will Build

Seven bilingual Jotform questionnaires

Each form should support English and Spanish while maintaining a consistent underlying data structure.

We do not want separate English and Spanish form architectures unless a genuine Jotform limitation requires it.

Advanced conditional logic

Applicants should see only the questions relevant to their answers.

Examples include conditional sections involving:

medications
treatment providers
previous employment
injuries
surgeries
symptoms
medical history

Changing an earlier answer must not leave obsolete hidden responses in the final output or cause hidden required fields to prevent submission.

Repeating / multiple-entry sections

Applicants may need to enter multiple:

medications
physicians/providers
previous jobs
injuries
surgeries
treatments
other historical information

PDF and structured data outputs

Each completed questionnaire must generate:

A readable completed-questionnaire PDF
Structured submission data suitable for our existing AI dashboard

You may need to coordinate with our dashboard developer regarding API, webhook, or JSON behavior.

You are not responsible for building the AI dashboard or medical-legal report-writing system.

General Specialty Questionnaire

One questionnaire covers approximately 15 medical specialties and must remain a single form.

Applicants will not select their medical specialty.

Instead, screening questions determine which detailed sections appear.

For example:

Do you currently have, or have you previously had, a skin condition?

If No, the detailed dermatology questions remain hidden.

If Yes, the applicable follow-up questions appear.

Important Data Requirements

The system must distinguish among:

No
I don’t know
Not answered
Not asked because the question was not applicable

A skipped question must not automatically be interpreted as a negative response.

Spanish free-text answers must be preserved exactly as entered.

Applicant Experience

The completed forms must be:

mobile friendly
easy to navigate
suitable for applicants with limited computer experience
primarily multiple-choice / checkbox based where appropriate
compatible with an approved save-and-resume workflow
Testing Requirements

You will be responsible for thoroughly testing your implementation.

Testing must include:

English and Spanish
mobile and desktop
conditional branching
changing previous answers
hidden fields
required fields
multiple/repeated entries
save and resume
PDF output
structured data / JSON output
final submission
consistency between questionnaire responses, PDF, and structured output

All development testing must use synthetic data only.

HIPAA / Security

These questionnaires will ultimately operate in a healthcare-related environment.

You must:

work within our approved company-owned environment
use synthetic data during development and testing
not place patient information into personal accounts
not use unauthorized third-party tools
Paid Technical Pilot First

We will begin with a paid technical pilot before awarding the full project.

Pilot budget: approximately $350

The pilot must demonstrate:

representative conditional branching
English/Spanish behavior
repeated/multiple entries
changing an earlier answer without retaining obsolete hidden data
PDF output
JSON/structured output
mobile usability

If the pilot is successful, we will proceed with milestones for the remaining questionnaires.

Required Experience

Strong demonstrated experience with:

Jotform
advanced conditional logic
multilingual forms
Jotform PDF Editor
APIs/webhooks or structured submission data
complex intake forms
troubleshooting and QA

Healthcare-form experience is preferred.

Scope Boundary

The workflow is:

You are responsible for the Jotform implementation, configuration, troubleshooting, and QA.

You are not responsible for:

writing medical questionnaire content
designing the AI dashboard
developing the report-writing agent
translating medical-legal reports
creating medical report paragraphs inside Jotform

We are looking for someone who can take a well-defined specification and implement it accurately and efficiently.
