Microsoft Truck Handover Form

I need a complete truck usage-and-handover system built in Microsoft Forms that automatically writes every submission to an Excel workbook. The form must capture the full set of data I rely on day-to-day: driver and truck details with the exact hand-over time; current mileage, fuel level, and a quick check on overall vehicle condition; plus space for damage photos, driver and supervisor signatures, and any extra notes.

Signatures must be drawn directly on a phone or tablet—no photo uploads or typed names—so the form has to use Microsoft Forms’ ink control (or another lightweight add-on) that still lands the image in the linked spreadsheet. Photos are required both before a driver takes the truck and again when it’s returned, making it easy to compare condition change; therefore two image fields need to be mandatory for each entry.

I want the answers streamed live into an Excel file stored in OneDrive or SharePoint, with clear column headers so later reporting is painless. If a quick flow in Power Automate is the best way to keep everything synced, please set that up too.
