Local Machine Contact Database

I'm looking for a contact information database, ideally built in Microsoft Access, for local machine use only.
This is a small project. The database will have about 50 fields or less, and most are simple text fields. They should all fit on one PC laptop screen.

The database needs to store:
- Names and phone numbers
- Email addresses
- Physical addresses
- Position and organization names
- Drop-down choice menus for categories; able to add new categories dynamically to the drop-down menus
- Unlimited-length miscellaneous notes fields, seachable on full text

Additionally, it should have:
- Search functionality
- Data entry forms
- Reports generation

Data will be inputted through manual entry and initially imported from an Excel file.

Ideal skills and experience:
- Proficiency in Microsoft Access
- Experience in database design and development
- Familiarity with data import processes from Excel
- Ability to create user-friendly data entry forms and generate reports (I will want to specify the layout on the data entry form, which will also be the primary record display format)
