I’m looking for a fill-able PDF that I can open on my iPad during new-construction inspections. The file must already contain roughly 50 common checkpoints I deal with every day, yet still let me insert additional rows when an unusual item pops up.

For each line I need two things: a simple checkmark box that instantly tags the item Approved or Disapproved, and a photo placeholder that lets me drop in (or capture) pictures on site. The photo should sit right beside its line so everything stays in context when I review the report later.

Because this has to run smoothly on an iPad, please build it with standard Acrobat-compatible form fields—nothing that relies on a separate app or web connection. Auto-expanding text areas for notes and a clean, printable layout will round out the workflow.

Deliverables
• One ready-to-use, fill-able PDF inspection sheet with 50 predefined items
• Functionality to add unlimited custom items, each with its own checkmark and photo field
• Tested on an iPad (Adobe Acrobat Reader or native Files preview) to confirm photos embed correctly and all checkmarks save
