Individual Net Worth Certificate (UDIN)

I need a Chartered Accountant who can issue an up-to-date Net Worth Certificate, complete with your official seal, signature and a valid UDIN. The certificate is for my personal bank-loan file, so it must follow the standard format that Indian banks expect and clearly reference me as an individual applicant.
