Employee Satisfaction Email Survey Design

I’m putting together a quick-hit email survey to gauge how my team feels about their workplace experience. I need 5–10 concise, multiple-choice questions that employees can answer in under two minutes, along with an email intro that encourages honest participation.

Here’s what I’d like from you:
• A well-structured list of 5–10 multiple-choice questions focused on key satisfaction drivers (communication, recognition, workload, growth).
• A brief, friendly email message that explains the purpose, assures anonymity, and links to the survey.
• The survey laid out in a share-ready format—Google Forms, Microsoft Forms, or Typeform are all fine; choose the one you’re most comfortable with and tell me why.

I’m open to mixing in a rating scale or one free-text question if you feel it will enrich the insight without adding length, but the core needs to stay multiple choice for quick completion.

Once the draft is ready, I’ll test it with a small group; if everything flows and the data exports cleanly to CSV, we’re done.
