Digitize 50-200 Paper Forms

I have between 50 and 200 paper forms that need to be keyed into a digital file. The information on each form is primarily text, with the occasional decimal number, and I need every field reproduced exactly as written—spelling, capitalization, punctuation, and numeric precision all matter.

I will scan the forms and send them to you as high-resolution PDFs. You can capture the data in Excel, Google Sheets, or another structured format we agree on, as long as columns match the headings on the forms. If something is illegible, flag it so I can clarify.

Deliverables
• A clean, fully-populated spreadsheet (xlsx or csv) containing one row per form
• A brief note listing any fields you flagged for clarification
