Each entry should show the date, time, booking reference, and a contact phone or email so I can reach the airline, hotel, tour operator, or restaurant in a hurry. A clean, easy-to-scan layout in a Word or PDF file is perfect; if you have a template you love, feel free to use it, otherwise build one that looks professional and printable.
