Custom Excel Patient Dataset

I have a backlog of patient files that need to be digitised and organised in a clean Excel workbook. The data includes two main sections: demographic information and medical history. For the demographic block I need columns that capture a patient’s name, age, complete contact information, address, and emergency contact. The medical-history section should leave room for concise notes on past conditions, medications, allergies, and any relevant observations.

Because I do not yet have a template, part of the job is to design one that is intuitive, consistent, and easy for non-technical staff to maintain. A little conditional formatting, drop-down lists for common entries, and locked formula cells to avoid accidental edits will be appreciated.

Deliverables:
• A reusable Excel template covering all required demographic fields and a clearly separated medical-history area
• The first batch of entries (approximately 100 records) entered accurately into the template
• Brief instructions so future staff can keep using and extending the file without breaking its structure

Accuracy and patient confidentiality are critical, so I will supply the source documents once we agree on basic layout and a secure hand-off method. If you have experience with healthcare data or HIPAA-conscious workflows, please highlight that in your response.
