Course Application Form Build

I’m launching a new course and need a straightforward online application form that I can link or embed directly into my existing website. The form’s core purpose is to collect a specific set of fields I already have defined; I’ll share those exact labels and any validation rules with you at the start of the project.

A key feature is a video upload step: every applicant must attach a 1- to 3-minute video introduction. Please build in server-side validation for file size and allow the common formats (MP4, MOV, or WEBM). If compression or transcoding is required for smoother storage or playback, let me know your preferred approach.

I’m flexible on the underlying platform—whether that’s a polished Google Form, a WordPress plugin, or a lightweight custom form that drops neatly into my site. I simply need something reliable, mobile-friendly, and easy for me to export or review submissions (CSV or similar).
