Convert PDF Form Data to Excel

I have a small batch of PDF forms—fewer than ten—that contain a mix of text and numerical fields. I need every field captured with absolute accuracy in an Excel workbook so the information can be filtered and analyzed later.

Please mirror the structure of each form: every unique field should map to its own column, and each completed form should appear on a separate row. Consistency in spelling, punctuation, and number formatting is essential because the file will feed directly into a reporting tool.

Deliverables
• One Excel (.xlsx) file containing all transcribed data
• A quick note summarizing any unreadable entries or anomalies you run into
