Clinical Video Data Entry

I have several video files and related clinical notes that need to be transformed into clean, well-structured digital records. Your task is to watch each video through to the ending, extract the key information I will highlight (patient ID, procedure timestamp, outcome remarks, and any numerical measures displayed on screen), and then enter those details alongside the corresponding notes into a single, consistent spreadsheet. Accuracy is critical because these records feed directly into an ongoing study, so every field must be double-checked for typos or misplaced numbers before you mark the row complete.

I will supply:
• A shared folder with the videos and scanned clinical sheets
• A template spreadsheet with the exact column order I need

You will deliver the completed spreadsheet plus a brief log describing any unclear entries you flagged for my review. Familiarity with standard spreadsheet shortcuts and a sharp eye for detail will be essential. If this first batch goes smoothly there will be follow-on sets of data to process.
