What is your impact?
The Care Coordinator works with administration, Care Managers, patients, and their family members to reach healthcare goals and keep the lines of communication open. Care Coordinator will support non-clinical duties and provide additional layers of support to the clinicians/case managers/members as it relates to patient care.
What Will You Be Doing:
Serves as a resource contact, education source to patients, families, providers, and/or staff
Provides telephonic follow-up with members to include post hospital discharge and chronic condition management
Completes assigned tasks related to Care Coordination
Generates tasks to the assigned RN Case Manager
Directs patients/families to appropriate resources including community resources
As directed and supervised by the RN Case Manager assists in the attainment of members goals including support, member education, access to resources, assisting in the arrangement with follow-up care, etc.
Maintains quality service by following organization standards
Assists with data entry, faxes, filing, and other clerical duties
Assists with handling inbound/outbound calls and obtaining information as requested by licensed staff
Qualifications
1+ years related work experience in a healthcare related role.
High school diploma or GED
Strong organizational and time management skills.
Excellent verbal/written communication.
Ability to communicate telephonically in a professional and efficient manner.
Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint).
Ability to multi-task in a fast-paced environment.
