I have a written synopsis that needs to be transformed into a clear, engaging online questionnaire. The finished form should be hosted on a popular platform such as Google Forms, Typeform, Microsoft Forms, or any equivalent you are comfortable with, as long as the shareable link works seamlessly on desktop and mobile browsers.
Structure & content
• Mix the three question styles I selected—multiple choice, short answer, and rating scale—so they flow naturally from the logic of the synopsis.
• Design each item to capture either text responses or numerical data, as appropriate. For example, rating scales should translate to clean numeric output while open-ended prompts record text accurately.
• Organise sections, progress indicators, and branching (if helpful) to keep respondents engaged and minimise drop-off.
Deliverables
1. Live form link, fully tested.
2. Editable copy of the form in its native platform.
3. Exported response sheet template (CSV or spreadsheet) showing how the captured text and numbers map to columns.
4. Brief note (max one page) explaining any conditional logic you applied and tips for future edits.
Success criteria
• Every question traces back to a point in my synopsis—no gaps, no extraneous items.
• Form loads fast and works on major browsers and phones without layout issues.
• Exported data is clean: text fields free of leading/trailing spaces, numeric fields validated for numeric input only.
