Brochure Update With Registration Page

I have an existing brochure that simply needs a refresh rather than a full redesign. Please update the body copy and all contact details throughout, then append one additional page at the very end.

That new page is a short registration form—no payment processing, just several fields like name, email address, and phone number. Keep the visual style completely consistent with the current brochure so the extra page feels native, not tacked on.

Deliverables I expect:
• Print-ready PDF of the full brochure, now including the registration page
• Original, editable source file (InDesign or Illustrator, whichever you prefer)

The layout is already clean, so this should be a straightforward text swap and single-page addition. Quick turnaround is appreciated.
