I need a small yet polished website whose sole focus is to gather service preferences from visitors before we confirm an appointment for them. There is no scheduling calendar or time-slot selection involved—the site’s job is strictly to collect useful data that helps my staff book the right service later on.

4. Code and setup instructions are supplied so I can redeploy or update the site myself.
