Description: ***Only apply when you have PROVEN knowledge of Liferay and Java portlet development (with links to previous work). Otherwise your application will be ignored. The payment will only be made when the portlet works in Liferay 5.0.0***

The portlet should follow the JSR 168 standard and will be used on both an internet and intranet Liferay environment (of the same website) - different questionnaires will be required for each environment though. Lay-out can be very basic but clean and a simple Microsoft Access DB can be used.

BACK-OFFICE
The administrator should be able to set-up a new questionnaire at any moment. A questionnaire consists of:
(1) questions (ex. "How do you value our services?")
(2) answers (on a scale from 1 to 5 ex. very bad - bad - good - very good- excellent) -> the answer descriptions could be different for each question of the questionnaire -> the administrator should be able to configure this.

The administrator should be able to set 1 questionnaire as the active questionnaire (this questionnaire will appear in the front-office). And the administrator should also be able to view the results of the previous and current questionnaires (just a textual presentation will do fine).

FRONT-OFFICE
Shows the active questionnaire title with the different questions and possible answers. When completed the form is send to the administrator and the results are saved in the Microsoft Access DB. Maybe a check can be build in that the same IP can only send in the questionnaire once a day.
